HOW TO PAY:
IN PERSON: Download the enrollment/application form. Bring your completed enrollment form to our office. Be sure to bring your state issued ID card and the required deposit (pay by: cash, personal check, credit card, debit card or cashiers check). Monday - Friday: 9:00 AM - 5:00 PMBY MAIL: You may mail your enrollment/application form with your credit card information, cashiers check, money order (made payable to Fast Track Health Care Education) or credit card information (Please remember to include a copy of your state issued ID and social security card for processing) and mail to:
